These forms are specific to the Psychology Department - these forms do NOT get returned to the Graduate College.

Please download the forms first then fill and sign.

Once they are completed, please return them to the Graduate Coordinator of the Psychology Department.

Evaluations:

  • TA Mid-semester Evaluation
    • Filled out by TA and instructor in the middle part of the semester 
  • TA Semester Evaluation
    • MANDATORY - Must be completed and signed at the end of each semester by the Instructor and TA 
  • RA Semester Evaluation (Fillable PDF) 
    • MANDATORY - Must be completed and signed at the end of each semester by the Instructor and RA
